Medical Billing for Nebraska Practices

Which Medicaid plans do Nebraska patients use?
Heritage Health is Nebraska’s Medicaid managed care program. In 2026 it covers physical health, behavioral health, dental, and pharmacy through three statewide plans: Molina Healthcare of Nebraska, Nebraska Total Care, and UnitedHealthcare Community Plan of Nebraska. The claim goes to the plan the member is enrolled in. Enrollment in Nebraska Medicaid alone does not route the claim.
How should a dual-eligible claim be sequenced?
When Medicare is primary, Medicare is billed first. Nebraska Medicaid, through the member’s Heritage Health plan, is billed second for what Medicare does not pay. A Medicaid claim sent before Medicare has adjudicated often denies or pends.
What is different about critical access hospital billing?
A critical access hospital can bill Medicare under the standard method or the optional method, often called Method II. Under the standard method, the hospital bills facility services and the clinician bills professional services separately. Under the optional method, the hospital bills the professional service only when that clinician has reassigned billing rights to the hospital. The claim has to match the method on file.
How Heritage Health routing differs
The three plans are statewide. A new member is enrolled in one plan and can change plans within 90 days of first enrollment, during annual open enrollment, or for an approved for-cause reason. The plan named on the member’s card is the plan that must receive the claim.
Nebraska DHHS says providers must enroll with the health plan before submitting claims and receiving payment for that plan’s members. A practice that is enrolled with one plan and not the other two can bill only the members of the plan that has loaded the provider.
Border-state patients
Nebraska clinics see patients who live in South Dakota, Iowa, Kansas, Colorado, Wyoming, and Missouri. Coverage follows the patient, not the clinic’s ZIP code. The same Grand Island office can have a Nebraska Heritage Health claim, a Medicare claim, a commercial claim, or another state’s Medicaid claim in the same week. Each one is a different path.
